Self-Exclusion
The Gambling Commission has developed a process where a person with a gambling problem or gambling disorder can submit a single form and voluntarily exclude themselves from all non-tribal casinos statewide. Tribal casinos may also participate in this statewide self-exclusion program. Please monitor this page for participating tribal casinos.
Individuals who want to self-exclude will need to complete the Self-Exclusion form you can download by clicking the link below.

Enrollment Appointment Calendar
Currently, the WSGC office is not open to the public and a scheduled appointment is required to enroll in person. Please bring proof of identity - a valid driver's license, government-issued ID with name, photograph, and date of birth, or valid passport.

Health Care Authority Transmittal Form
The transmittal form is only for use by house-banked cardrooms to transmit confiscated funds to the Washington State Health Care Authority’s Problem Gambling account in accordance with RCW 9.46.071 and WAC 230-23-030.
