Self-exclusion program enrollment

The Gambling Commission has developed the voluntary Self-Exclusion Program for individuals with a gambling problem or gambling disorder. The Self-Exclusion Program allows you to submit a single form to voluntarily exclude yourself from all house-banked cardrooms statewide. Tribal casinos may also participate in this statewide Self-Exclusion Program. Please monitor this page for participating tribal casinos.

There are three options to complete your application:

  1. Complete the Self-Exclusion Enrollment application at home and mail it to WSCG Headquarters in Lacey
    1. Fill out a Self-Exclusion Enrollment Form. Translated Self-Exclusion Enrollment Forms are also available.
    2. Your enrollment form must be notarized or signed by a certified gambling counselor.
    3. Attach a passport style photo and a copy of your photo ID to your application.
    4. Mail your complete application to:

  2. Complete the Self-Exclusion Enrollment application at the Washington State Gambling Commission Headquarters
    1. Book an Appointment Online to complete the Enrollment Form and have your photo taken at the Washington State Gambling Commission Headquarters in Lacey, WA.
    2. Bring proof of identity to your appointment:
      1. Valid driver's license.
      2. Government-issued ID with name, photograph, and date of birth.
      3. Valid passport.
  3. Enroll in the Self-Exclusion program at a house-banked card room
    1. Visit any licensed house-banked card room in Washington State.
    2. Request the Self-Exclusion Enrollment Form from staff, complete it, and return it to a house-banked card room employee.
    3. The house-banked card room will mail your form in. Your enrollment date starts the day you give your enrollment form to the house-banked card room.

Self-exclusion expiration and removal

You will receive a notice 45 days before the expiration of your voluntary self-exclusion period. Once your voluntary self-exclusion period has expired you are required to send in a notarized Self-Exclusion Program Removal Form to be removed from the program. You will remain in the program until we receive and process your Removal Form.

    1. Fill out a Self-Exclusion Program Removal Form.
    2. Your Removal Form must be Notarized.
    3. Mail your complete application to:

  2. Complete the Self-Exclusion Removal Form at the Washington State Gambling Commission Headquarters

  • Health Care Authority Transmittal Form. The transmittal form is only for use by house-banked cardrooms to transmit confiscated funds to the Washington State Health Care Authority’s Problem Gambling account in accordance with RCW 9.46.071 and WAC 230-23-030.
